Total Budget: $5,000

Area of Focus: Basic education and literacy

Summary: Title one school - Providing School Uniform Shirt, Supplies for Middle School

Whitney Elementary School is a Title one school. Lots of poor immigrant children. Many are very anxious about affording the uniform shirts and backpack/supplies that are required for their transition to South Middle School.

Recently the sixth graders collectively put together a book. We plan to pay for printing and binding so every participant gets a bound copy.

For the 60 6th graders we also are providing a 1)required polo uniform shirt $15 2) Backpack donated by Bucks Bags $15 3) plus miscellaneous school materials $30. $45 per student x 60 = $5000 Total expenditure $5000

Rotarians are directly involved with planning, acquiring the materials, filling the packs and presenting them to the students.

Your application must be signed by the project manager and the 2022-23 club president for it to be considered for funding as part of the district's block grant from The Rotary Foundation. If you have no changes to make to your application, please start the club signature process on the administration side of the description tab in your grant application. Your district grant project has been included in D5400's block grant application to The Rotary Foundation. It is not yet approved. Do not place orders or spend money on the project until you hear from the district that TRF has approved our block grant (likely sometime this month), as you cannot be reimbursed for expenses incurred prior to TRF approval. Call Marianne Barker at 208 308-5617 if you have any questions. Thank you!

A grant report is due no later than May 15th, 2023. Failure to report by the deadline may impact your club's ability to receive a grant in 2023-24 Rotary year. For final report, download the report form from the administration side of the description tab. Fill it out, being sure that the expenses listed in the report match your receipts and any other documentation of expenses you provide. Upload pdf copies of your report and the receipts to the documents tab of your grant. Then click "reported", a button on the administration side of the description tab. If the project is not done, file a progress report - a history log telling us when you started the project, what % complete you are & when you plan to finish it out. Call Marianne 208 308-5617 if you have questions about reporting.

This is a reminder that you have a report due on this grant by May 15th, 2023. If the project is completed your submittal should be a final report. You'll find the report form on the admin side of the description tab for download. Complete the report, save as pdf and upload to the documents tab along with supporting receipts. If project is not complete, provide a progress report by sending a history log email stating when you started the project, what percent complete it is and when you anticipate being wrapped up. That's all we need for a progress report. Failure to report may impact your club's ability to receive a grant in the 2023-24 cycle, so please get your report in. Call Marianne 208 308-5617 if you have questions or need help.

Today, May 15th, is the deadline for reporting on 2022-23 district grants. Failure to report may cause your club to be ineligible for a grant in the 2023-24 cycle. If your project is ongoing, a simple progress report will suffice. In your grant, enter a history log stating when the project started, what percent complete it is and when you anticipate wrapping it up. If you've finished out the project your final report is due today.

I downloaded the "report5400.doc" form but it would not open as it was corrupted.

The club spent 3599.43 towards this project for backpacks with school supplies for Title One 6th grade students of Whitney Elementary to transition to Middle School. The backpacks were well received and appreciated. Receipts were uploaded.

We were able to download the final report form from your grant on matchinggrants.org. The file is not corrupted. A history log is inadequate for filing a final report. Please download the Word document from the administration side of the description tab and complete & file your final report. Completion of your reports on this and past-due grant P-3364 is critical to have your application be considered for inclusion and funding in the 2023-24 block grant application to The Rotary Foundation from District 5400. Thank you. Call Marianne at 208 308-5617 if you have questions or need help.
