Rotary Club: Chula Vista-Eastlake Project Title: Youth Service Chula Vista 1. Describe the project. What was done, when and where did project activities take place? Supported 2 students to go to LEAD on October 14-15. Camp organize by District 5340 Supported 2 students to go to RYLA on April 20-22 Camp organize by District 5340 2. How many people benefited from this project? 6 3. Who were the beneficiaries, how were they impacted by this project, and what humanitarian need was met? 4 students that got to experience a one in a life time opportunity to attend a motivational camp that will help them excel for excellence in there school, family and life (LEAD 2 students and RYLA 2 student); helping by promoting basic education. 4. How many Rotarians participated in the project? 6 5. What did they do? Please give at least two examples, not including financial support provided to the project. Several facilitators (1 at LEAD and 1 at RYLA). 1 rotarian was the direct contact with LEAD and RYLA 1 rotarian was the direct contact the Model UN 6. District must retain receipts of all expenditures for at least five years. Please scan and upload to the project's "Documents" tab all invoices (be sure they are marked "paid"), receipts and/or copies of checks for the full amount of the project budget.
