Rotary Club: Chula Vista-Eastlake Project Title: Hearts For Arts 1. Describe the project. What was done, when and where did project activities take place? Provided donations the the Chula Vista Visual and Performing Arts Elementary School district project, so that students can overcome family and personal problems using the support of the performing arts 2. How many people benefited from this project? All the students in the elementary level of Chula Vista 3. Who were the beneficiaries, how were they impacted by this project, and what humanitarian need was met? The students on the elementary level; by supporting the visual and performing arts, students get to express, learn and be able to have better means to develop skills for there future, be able to overcome adversities and be more productive at school 4. How many Rotarians participated in the project? 15 5. What did they do? Please give at least two examples, not including financial support provided to the project. Organize an event to promote the work the visual and performance arts program does for the elementary school district of Chula Vista 6. If a cooperating organization was involved, what was its role? Part of the organization committee of the promotion of the organization and its benefit for the community 7. Income

District must retain receipts of all expenditures for at least five years. Please scan and upload to the project's "Documents" tab all invoices (be sure they are marked "paid"), receipts and/or copies of checks for the full amount of the project budget. By signing this report, I confirm that to the best of my knowledge these District Grant funds were spent only for eligible items in accordance with Trustee-approved guidelines, and that all of the information contained herein is true and accurate.
