Rotary Club: North County San Diego-Route 78 Project Title: Hope for Mtshabezi Clinic 1. Describe the project. What was done, when and where did project activities take place? This grant was to support a medical clinic in Zimbabwe that our club has been supporting for several years. This project was to supply funds in order to purchase medical supplies, primarily medicine. The clinic is the only one is this region of Zimbabwe. 2. How many people benefited from this project? Hundreds 3. Who were the beneficiaries, how were they impacted by this project, and what humanitarian need was met? The clinic is the only facility in the region, and provide medical services to many who otherwise would not have access. Without this clinic, hundreds if not more would be in a situation of medical insecurity. 4. How many Rotarians participated in the project? 1 5. What did they do? Please give at least two examples, not including financial support provided to the project. Provide needed medicines to those that need it. Provide needed vaccinations to those that need it. Provide general medical services for those in the area. 6. If a cooperating organization was involved, what was its role? No other organization. 7. Income

District must retain receipts of all expenditures for at least five years. By signing this report, I confirm that to the best of my knowledge these District Grant funds were spent only for eligible items in accordance with Trustee-approved guidelines, and that all of the information contained herein is true and accurate.
