Crisis Nursery Vehicle

A Rotarian in the Lilongwe Club and nis wife started a Nursery for infants in crisis, mostly malnourished (infant mortality rate in Malawi is 20%). This was started in their home and has now expanded to larger quarters due to demand and need...

A vehicle is needed to transport the infants to and from their homes in the country and to assist care givers in their needs and to transport supplies and food.

Emailed Grant Agreement form to Dean Lungu and Debbie Day for their signatures as president. Also, emailed Payee Form to McDade and Kamisa to complete the bank account info. Once returned to me I will forward them to Jennifer Berg who said they needed to be completed. The funds from the host and international clubs have been deposited as well.

Joe Kamisa emailed the payee and grant agreement forms, and Brennan Pate emailed the San Diego grant agreement form to Jennifer Berg to fulfill all paperwork requests.

Jennifer Berg wrote that she received paperwork and grant will be forward to payments department for payment. The funds (50,150 USD) should arrive in the project account in Malawi within 4-6 weeks.

Joe Kamisa received permission from Jennifer Berg to use the remaining funds (~$4000) in and around the Crises Nursery, pending agreement by international partner.

Jennifer Berg wrote that she cannot accept the report as a "Final Report" becasue there is still a balance of 580,153 MWK left to report on therefore this can not be a Final Report unless the funds are returned to the foundation. As stated before the funds may be spent on eligible items related to the original grant and then report on how those funds were spent in order to close out the grant. Please note that the next report on this grant will be due on 3-December-08 or within two weeks of project completion, whichever occurs first. In addition, any subsequent reports must be signed by RC Lilongwe's current club president.

Jennifer Berg wrote that the certifying signature on the report form confirming that grant funds were spent in accordance with Trustee guidelines was incorrect or incomplete. Need to submit a copy of report appropriately authorized by the current Club president of either the host or international partner. (President Bailey or President Warnke) Authorization should be an actual signature. However TRF does not require an original signature. Therefore the signature may be faxed or scanned and emailed.
