   Ukraine - Dneprodzerzhinsk

Dneprodzerzhinsk Hospital

This project will work with the ROTARY club of Dneprodzerzhinsk order to help provide equipment to a poorly equipped surgeons working in a regional hospital which is in charge of taking care of the surgical and dental needs of the areas orphan and indigent pediatric population afflicted with the birth defects of Cleft Lip and Palate and Craniofacial deformities. The length of time for equipping and training the staff on the use of the equipment is estimated to take approximately two or three years.

Ukraine doctors hve been screened and two have been selected to come over to the United States for exposure to the use of the intended equipment and the modern techniques used for the children's deformity corrections. Separate funds have been obtained for this purpose and the selected doctors have been given permission from their directors of the hospital departments for this training. They will be given contracts from our funding sources here in the USA for their participation requiring them to give at least 50% of their time to the charitable efforts at the hospitals upon their returns for the first two years.

The club will work with the orphanages for continuing site management as well as help the coordination of the screening plans.They help in the logistics of the management and instructive teams during the missions visits twice per year. Interpreters will be provided and drivers for logistics as well as assistance in customs issues.

International Partner Participation- specify what the club's non-financial activities are. (Mission Bay RC)

Ownership- Please explain who Dr. Anotoly Komok is and why the equipment is not being donated directly to the hospital's ownership.

Host Committee and Authorizations- Conflict of interest: Rotarian Moses is also the President and founder of the cooperating organization, in order to avoid the appearance of a conflict of interest Rotarian Moses should be removed from the project committee. As a result please:

1. One additional active member of Mission Bay RC should send Jennifer an email or letter stating that he/she will oversee this project for its duration and we will accept this, in order to bring the total committee up to 3 members.

Cooperating Organization Letter- The letter also must state that it agrees to cooperate in any financial review of activities connected with the project. Please provide a new letter that contains this language.

Rotary Endorsement of Cooperating Organization-
President of RC Dneprodzerzhinsk needs to provide a signed letter from the host club that endorses the cooperating organization and states that it is reputable, responsible, and is registered and acts within the laws of the project country. Jennifer is also able to accept an email from President Polyalov's email account.

Berg also allocated Hunter's contribution to the Mission Bay RC and Rotarian Moses' contribution to RC Carlsbad.

Dr. Moses submitted to Jennifer Berg the following per her reuquest: -Revised budget and modified equipment and financing to match appropriately. -Revised grant proposal. -A scanned a signed copy of the supporting organization agreement -sample letter of Supporting Organization Verification for the President of the Dneprodzerzhinsk Rotary Club to revise and sign to return to Jennifer Berg.

Dr. Moses has submitted funding from the Ukraine Club and his own contribution through the Carlsbad Rotary Club. Checks from Tricia Hunter and the Mission Bay Rotary Club Foundation have been sent in today along with another copy of the signed agreement.

A complication regarding the $100 contribution from the Ukraine Club has resulted in a delay of funding for this grant. Dr Moses has been in contact with the Ukraine Club and TRF to resolve this issue.

Jennifer Kordell confirmed receipt of the $100 contribution sent via Western Union from the Ukraine. Grant payment will be distributed once processing is complete.

Several pieces of the equipment originally identified for purchase have been donated to meet the needs of this grant. Additional equipment is being purchased and will be transported to the Ukraine in the next few months.

The trip to the Ukraine occurred from May 9 to 18, 2008. The purchased equipment was presented to the hospital on the 12th of May and 17 surgeries were performed with the new equipment. A final report has been prepared and was signed by the President of both participating clubs.
