Puentes de Esperanza (TB)

Puentes de Esperanza (Bridges of Hope) is a binational alliance of providers and organizations dedicated to developing capacity to treat multi-drug resistant tuberculosis (MDR-TB) patients in the California-Baja California region. MDR-TB threatens communities world-wide. It is difficult and expensive to cure, and the resources to fight these strains do not exist in many areas of the world. In Baja California an estimated 50-100 individuals are afflicted. Unable to get curative treatment, MDR patients in Baja California become increasing ill over a period of years during which they can spread disease within border communities and areas in the interior of both countries.This binational consortium will aim to create the infrastructure to support early recognition of affected patients and outreach services to assist patients with successful adherence to treatment. The expected duration of the project is 3 years.

The Puentes de Esperanza project will develop a binational model for sharing professional expertise and demonstrate success in providing access to vital services for patients with the most difficult forms of TB. Training and education of providers and health care workers, and creation of a sustainable cross-border exchange of expertise are primary program objectives. Establishing this model will assist Mexican health authorities apply to the World Health Organization's "Green Light Committee" to obtain relatively inexpensive drugs to treat MDR-TB patients and will improve the overall capacity of the Baja California TB program to prevent and control TB.

Funds from this matching grant will support a variety of value added activities of Puentes de Esperanza which would otherwise not occur including binational meeting workshops for TB care providers, patient incentives such as food and transportation, protective masks for outreach workers, and production of public service annnouncements about tuberculosis. The international and host partners will visit the project site at the Tijuana Health jurisdiction. The host and international partners will share information about project progress via email correspondence and joint meetings. The host and international partners will also seek media coverage for the project in the community and in the District.

The Puentes de Esperanza project with support from Rotary is progressing well. 12 patients with drug-resistant TB have been enrolled; 1 has completed treatment successfully, 11 are receiving ongoing treatment and 8 of these patients have converted to negative sputum cultures meaning their treatment appears to be working well. Rotary has contributed to the project so far by providing incentives such as transportation for enrolled patients to come to the TB clinic services. Rotary has also supported the purchase of protective masks for health workers including promotoras who work closely with these patients when they are still infectious. Trainings in infection control and the use of the protective masks have been given 27 individuals involved in the project in Mexico.
